Applying for CAP15189 Accreditation

Initiate the CAP15189 Accreditation Application Process

1. Complete the request for application form.
2. A College of American Pathologists (CAP) representative will contact you with more information about the program based on your laboratory's needs.
3. You will be asked to provide a completed application along with documentation that describes your laboratory's quality system.
4. The CAP will then assign a lead assessor who will guide you through the subsequent steps to become accredited to ISO 15189.
